In addition to providing the services outlined in your customized care plan, we will have a Nurse Supervisor visit on a regular basis to evaluate your continuity of care, as well as your satisfaction with our services.
Your Client Care Coordinator will continue to monitor you to make sure your needs have not changed. We also stay in touch with caregivers and health professionals, keeping them informed of any changes in the care we are providing you. Above all else, we’re always just a phone call away.
